Insane Mode

- Hard mode simply wasn't enough, insane mode has been added
- This is what hard mode used to be and what the controls were originally when I first created the game
- It is definitely harder than hard and easy mode, but also definitely not impossible
- New corresponding achievements: Insane and Hacker have been added
